Intrauterine Insemination (IUI) 

IUI offers a less invasive first step for many women struggling to conceive after borderline tumor treatment. This procedure involves placing sperm directly into the uterus during ovulation, bypassing potential cervical factors and increasing the chance of fertilization.

For women with one remaining ovary and patent fallopian tubes, IUI combined with ovulation monitoring can be an effective and relatively low-cost option before considering more intensive treatments.

Ovarian Stimulation Considerations

The relationship between fertility drugs and borderline tumor risk remains debated. Some studies suggest associations with clomiphene use (a medication used to stimulate ovulation) while others find no clear causation.

Serous borderline tumors: These tumors recur more frequently but when they do recur, they typically remain borderline rather than becoming invasive cancer. However, the micropapillary variant deserves special attention — it’s more likely to present with invasive implants at diagnosis and may warrant more aggressive initial treatment or closer follow-up. When serous borderline tumors do have invasive implants, they’re usually the micropapillary type.

Bilateral cystectomy, or USO plus contralateral cystectomy, is often required in cases with bilateral involvement. 

Mucinous borderline tumors: Usually unilateral, these tumors allow for straightforward USO in most cases. However, when recurrence occurs, particularly outside the ovary, the risk of invasive transformation is higher. Some experts recommend completion surgery after childbearing for mucinous types with intraepithelial carcinoma or microinvasion.

Decision-Making and Long-Term Considerations

Several factors guide fertility preservation decisions in borderline ovarian tumors. 

Age and reproductive goals matter most — women under 35 with strong fertility desires often benefit most from conservative approaches. 

Tumor characteristics including stage, histologic subtype, and high-risk features like micropapillary pattern or microinvasion also influence treatment choices.

The extent of disease and ability to achieve complete removal affect surgical options, while conservative management requires committed long-term surveillance.

After childbearing, whether to remove the remaining ovary and uterus remains debated. Many experts favor watching serous tumors without high-risk features, intervening only if recurrence develops. For mucinous tumors with concerning features or microinvasion, removing the remaining reproductive organs may be recommended.

The psychological impact of living with higher recurrence risk shouldn’t be overlooked. Ovarian cancer support groups and counseling provide valuable resources for navigating these challenges.

The Impact of BOTs on Fertility 

Research continues to refine our understanding of borderline ovarian cancer and fertility preservation. Better molecular markers may eventually predict which tumors are most likely to recur or progress to invasion.

Improved fertility preservation techniques, including advances in egg and embryo freezing, continue expanding options for women facing these diagnoses.

The key message remains hopeful: most women with borderline ovarian tumors can preserve fertility while maintaining excellent survival outcomes. The combination of conservative surgery, close follow-up, and assisted reproductive technologies when needed offers multiple paths to motherhood.
