How long does surgical menopause last? Technically, forever.
The symptoms are a separate question, and the honest answer is more complicated than “they fade.” Here’s what the research shows:
- Hot flashes climb fast. Three months after risk-reducing salpingo-oophorectomy (RRSO), hot flash prevalence jumped from 5.3% to 56.2%, and night sweats from 20.2% to 47.2%.
- Then they plateau, not decline. At 24 months, vasomotor symptoms sat at 59% and night sweats at 39%. Symptoms peaked by three months and stayed stable across two full years.
- Without hormone replacement therapy, improvement is unlikely in year one. Most women not on HRT reported the heaviest symptom profile at 3 months, with only a 2% chance of improving by 12 months. Among HRT users, two-thirds had fewer symptoms and far better odds of improvement.
- Some symptoms persist for years. Research found that 69% of women who had RRSO before age 52 reported moderate or severe symptoms on the Menopause Rating Scale an average of 7.9 years after surgery. Of those, 57% reported severe urogenital symptoms.
So no, this isn’t a storm you wait out. What tends to change is how disruptive the symptoms feel and how well they’re managed, and that part you have real influence over.

What Is Surgical Menopause?
Surgical menopause happens when both ovaries are removed: a bilateral oophorectomy, often performed alongside a hysterectomy.
The ovaries are your main source of estrogen and progesterone, and they produce roughly half of your circulating testosterone — the adrenal glands supply the rest.
Take the ovaries out, and estrogen and progesterone drop almost overnight instead of tapering over years the way they do in natural menopause. Testosterone falls too, though not to zero, since the adrenal contribution stays.
That’s the short version, and it’s all you need to make sense of the timeline below. Surgical Menopause: How Long Does It Actually Last?
Here’s the part that catches a lot of women off guard: surgical menopause itself is permanent. Once both ovaries are removed, there’s no “getting through it” to the other side, the way people sometimes talk about waiting out a rough patch.
Worth being precise here, though: natural menopause is permanent, too. Menopause itself is technically a single point in time — the one-year mark after your final period — and postmenopause is every day after that, symptoms included. Nobody comes out the far side of it.
The real distinction is with treatment-induced menopause. Chemotherapy or ovarian suppression can shut down ovarian function temporarily, and for some women, particularly younger ones, the ovaries start producing hormones again months or even years later. Surgical menopause offers no such possibility. The ovaries are gone.
But permanent doesn’t mean unchanging. Symptom intensity tends to follow a rough pattern, even though every woman’s timeline looks a little different.
The First Few Weeks: Not the Peak
Symptoms often start within days, sometimes even hours, of surgery. That part is real. What the data doesn’t support is the idea that this is the worst of it. In one study, hot flash prevalence went from 5.3% at baseline to 56.2% at three months, meaning symptoms were still building well past the early weeks.
So what you feel in the first few weeks is a starting point, not a peak. Hot flashes, night sweats, mood swings, and exhaustion can all show up quickly, layered on top of surgical recovery itself, which makes them hard to separate from the surgery.
Anesthesia, pain medication, and interrupted sleep produce their own fatigue and fog. And the full list of menopausal symptoms runs well past 40, so plenty of what you notice in these weeks may not read as “menopause” at all.
If symptoms feel manageable at first, that’s genuinely good, but it isn’t a reliable signal for what month three looks like. Worth knowing before you decide whether you need a symptom management plan.
Months One Through Six: The Peak
This is the window where symptoms are still climbing, not easing. Vasomotor symptoms peak around the three-month mark and then hold steady, so if hot flashes and night sweats intensify during this stretch, that’s the expected pattern, not a sign something went wrong.
It’s also when most women start hormone replacement therapy. According to research, 61% of women initiated HRT after surgery. More than half started within the first week, and 79% were on it within three months. Systemic estrogen tends to work quickly, with relief in weeks rather than months, so if you started HRT soon after surgery you may already be feeling the difference by the time symptoms would otherwise be peaking.
Two things worth being clear about, though. HRT reduces vasomotor symptoms but doesn’t eliminate them. At 24 months, 54% of HRT users still reported vasomotor symptoms, compared with 88% of non-users. And for many women in our community, HRT isn’t a straightforward option at all. From Six Months to Two Years, and Beyond
Mood tends to stabilize somewhere in this stretch, and for most women the symptoms become more predictable, which is its own kind of relief. But predictable isn’t the same as gone. Vasomotor symptoms stay stable through 24 months, with 59% of women still reporting them two years out.
So what tends to change here isn’t the symptoms so much as how you live around them. Most women have settled into a “new normal” by this point, but that’s adaptation, not resolution.
Genitourinary symptoms are the exception, and not in a good way. Vaginal dryness and low libido don’t just linger, they tend to worsen and persist indefinitely without treatment. This is the piece most likely to still be affecting you years out, and also one of the more treatable. Vaginal estrogen is safe for most women with ovarian cancer. Raise it with your oncologist early rather than waiting to see if it resolves on its own.
Why Surgical Menopause Can Feel More Intense
Natural menopause gives your body years to adjust to declining hormones. Surgical menopause skips that runway entirely. One day your ovaries are producing hormones, the next day they’re gone.
Research backs this up. A widely cited review found that abrupt loss of ovarian function is linked to more severe outcomes than natural menopause, including higher rates of cardiovascular disease, cognitive changes (dementia risk and cognitive decline), and bone loss in women who go through it before their expected natural menopause age. Research also notes that bone loss can reach up to 20% in just the first 18 months after bilateral oophorectomy.
Surgical menopause can also bring more severe vasomotor symptoms, sexual dysfunction, and increased cardiovascular and osteoporosis risk than a natural transition, largely because the hormonal drop is so sudden.
If you had a hysterectomy as well, your body is also healing from major surgery at the same time it’s adjusting hormonally.
That double workload is real, though it’s worth separating the two. Adding a BSO to a hysterectomy isn’t associated with a more difficult surgical recovery or more complications. What it adds is the hormonal drop layered on top of normal healing, which can make those early weeks harder to read.
What Affects How Long Symptoms Last
No two women experience surgical menopause the same way. A few factors tend to make a real difference:
- Age at surgery. Younger women, especially those in their 30s or early 40s, often experience more intense symptoms since their bodies weren’t naturally preparing for menopause yet.
- Whether HRT is used. Hormone replacement can significantly shorten how long symptoms feel disruptive, though it isn’t right for everyone, especially for LGSOC patients.
- Type of surgery. Removal of both ovaries causes an immediate, sharp transition. Removing only one doesn’t trigger menopause at all, which is exactly why surgeons preserve an ovary when they can. The remaining ovary keeps working, though research suggests natural menopause tends to arrive about 1.8 years earlier than it otherwise would.
- Overall health and support. Nutrition, movement, and emotional support all play a role in how the body copes with the hormonal shift.

Managing the Symptoms While They Last
You don’t have to just wait it out. A few approaches worth discussing with your care team:
Non-Hormonal Options
For women who can’t take HRT, there are real alternatives. Fezolinetant (Veozah) and elinzanetant (Lynkuet) are FDA-approved NK receptor antagonists that target the brain’s temperature regulation directly rather than working around the hormone loss. NCCN guidelines list elinzanetant as a preferred non-hormonal option, along with gabapentin and venlafaxine.
Hormone Replacement Therapy (HRT)
For women who go through surgical menopause well before the average age of natural menopause (around 51), major menopause societies generally support offering HRT until roughly that age, unless there’s a specific reason not to.
The reasoning is that these women are losing years of hormone exposure their bodies were still expecting. A woman who has surgery at 35 loses roughly 16 years of ovarian function compared to the average natural menopause around 51. HRT can meaningfully ease hot flashes, protect bone density, and support cardiovascular and cognitive health.
That’s the general guidance. The exceptions matter, though, and for many women in our community, they apply.
Low-grade serous ovarian cancer is frequently hormone-receptor-positive, meaning estrogen can act as fuel for the tumor. That’s not a fringe theory: it’s the entire basis for treating LGSOC with endocrine therapy in the first place. Drugs like letrozole and other aromatase inhibitors work by lowering estrogen.
A personal history of hormone-receptor-positive breast cancer is another major contraindication, and that matters especially for BRCA carriers, who may face risk from both cancers. For BRCA carriers without a breast cancer history, though, NCCN guidance is that HRT is generally not contraindicated.
- Ask about receptor status specifically. Whether your tumor was ER/PR-positive is one of the biggest factors in this decision, and it’s in your pathology report. If you don’t know it, ask for it.
- Vaginal estrogen is a separate conversation from systemic HRT. And a much less contested one. Low-dose vaginal estrogen delivers a fraction of the systemic exposure, and NCCN survivorship guidelines include it as an option for survivors of hormone-sensitive tumors, with rings and suppositories preferred over creams because absorption is lower. It’s still an individualized decision to make with your oncologist. But the real debate is around systemic HRT, not this.

Lifestyle Measures
Regular exercise, a diet with adequate calcium and vitamin D, reducing alcohol and caffeine, and stress management can all support your body through the transition.
Weight-bearing exercise in particular helps bone density, and it’s one of the few recommendations that applies to essentially everyone.
But lifestyle alone won’t protect your bones after surgical menopause. You’ll likely need routine DEXA scans to track bone density, and some women need medication on top of that. Ask your care team when your first scan should happen.
Targeted Symptom Care
Vaginal estrogen can help with dryness and discomfort even in women who can’t use systemic HRT. Vaginal DHEA (dehydroepiandrosterone, a topical treatment) is another option, but it comes with a caveat: NCCN survivorship guidelines note that data in survivors of hormonally mediated cancers is limited, so safety isn’t fully established.
DHEA also converts to estrogen through aromatization, which is worth raising if you’re on an aromatase inhibitor. Like vaginal estrogen, it needs your oncologist’s sign-off. If intimacy has become difficult, our guide on ovarian cancer and sex has practical guidance.
You Deserve Real Answers, Not Guesswork
Because surgical menopause is permanent, long-term health monitoring matters just as much as symptom relief in the early months.
Bone density scans, cardiovascular check-ins, and cognitive health conversations with your doctor should become a regular part of your care, particularly if you went through surgical menopause before age 45. Research links oophorectomy before that age without HRT to increased all-cause mortality, and these risks don’t fade as you get further from surgery. Without intervention, they compound.
Surgical menopause is permanent, and so are some of its symptoms. But permanent doesn’t mean unmanaged. What matters most is having a care team that takes your symptoms seriously and a plan tailored to your history, whether that includes HRT, non-hormonal treatment, or a combination of both. And that plan isn’t something you settle once and forget; it’s worth revisiting as your body, and your needs, change over time.
